Booking form

A short form for your Contact page that captures the prospect's details first, then lets them pick a slot on your closers' calendars. Set its questions and install it in a minute.

What it is

The booking form is the Website Agent's second place on your site, next to the chat widget. It is meant for your Contact page: the prospect leaves their details, then picks a time slot, without going through a conversation.

It shares the essentials with the widget:

  • the same time slots and closers (duration, notice, assignment),
  • the same blocking rules (blocked IP addresses, emails, countries and dialling codes),
  • the same destination: the contact in Contacts, the meeting in Bookings.

To open it, go to Agents › Website Agent and, on the Booking form line of On your site, click Configure. The first time, this creates the form. Only an admin can create it.

What the prospect fills in

Their phone number

The form asks for the phone number first, and it is required, then the first name and last name.

Then the form unfolds

Once phone, first name and last name are filled in, the rest appears: email, your own questions and consent. When the prospect clicks Continue, everything is saved, even if they stop before booking.

The time slot, alongside

The calendar is visible next to the form but locked until the form is filled in. The first free day is pre-selected and times are offered every half hour. The prospect picks a time, then confirms it.

Phone, first name, last name and email are always asked, in that order. The questions after them are yours.

Your questions

In the What the prospect fills in card, click Edit the form:

  • + Add a question adds a question. Write its label (up to 80 characters).
  • Answer type: Text (one line) or Open text (a paragraph).
  • Answer required: tick it to make the question mandatory.
  • Move up and Move down reorder the questions; Remove this question deletes one.
  • Click Save to keep your changes, or Cancel.

You can add six questions at most: a contact form is not a survey. By default, the form asks for the company (required) and an open question about what brings the prospect.

Keep the form short. Every extra question costs completions, and the closer can ask the rest during the call.

Time slots and closers

The Time slots and closers card summarises what the form offers: the meeting length, the minimum notice and how meetings are shared between your closers (in turn, or by results). These are the same as the widget's. To change them, click Edit in Availability. See Forms and scheduling.

After the form

  • Those who stop halfway: the prospect's details were saved at Continue, so the Nurturing Agent can have a closer call them, then follow up by email if they do not answer.
  • Those who book a meeting: the Handoff Agent briefs the closer, just like for meetings booked in the widget. Since there was no conversation, the brief is built from the prospect's answers and notes that they booked without going through the chat.

Each card links to the agent's settings. See Agents.

Preview

The right side shows a Live preview of the form. Switch the device between Desktop and Mobile, and the step between Form and Time slot. Opening the preview launches the real form in a new tab, in test mode: nothing you do there counts.

If the form is switched off, its public page shows nothing, and the preview says so.

Install it on your site

The Nudge script is already on your site if the widget is installed. The booking form only needs one extra tag where the form should appear. Click Install on my site, then choose Where to show it:

Paste this tag on your Contact page, where the form should appear:

<div data-nudge-form="your-form"></div>

Choose Your site (Framer, WordPress, Webflow or HTML code) to get the right instructions: an Embed block in Framer, a "Custom HTML" block in WordPress, an Embed element in Webflow, or directly in the page's HTML.

The dialog shows the exact code for your form: use Copy the code, not the examples above, which use a placeholder.

Check the installation

Publish your page, open it once in your browser, then click Check the installation. Once Nudge has seen the form on your site, the status turns to Installed and the header says on which page it was detected (for example "Form detected on your Contact page").

If your site has a Content Security Policy that blocks Nudge, the dialog lists what to add. See Content Security Policy.

The tag needs the Nudge script on the same page. If the widget is not installed on your site yet, install it first: see Installation.
